Case of Duchhene Muscular Dystrophy (DMD)

CHIEF COMPLAINT:-

A 4 year child brought by parents with complaint of

  • Delayed walking with
  • fall while walking &
  • Weakness in legs & calf muscle ( calf muscles bulky , hard )
  • Started since 1 year back & diagnosed as a Duchenne’s muscular dystrophy. Mother lifts him up while walking.

FINAL DIAGNOSIS:- DUCHENNE’S MUSCULAR DYSTROPHY

 MILESTONE:- He was FTND & all vaccinations were given .all his milestones were normal except walking which was delayed , he started walking at the age of 2 & half yrs & then recognizes the swelling of calf muscle  & fall while walking .& pain In legs . Weight at birth was 7 pound.

MOTHER HISTORY DURING PREGNANCY:-

  • She wanted male child only, because in her family there is no male child that’s why she had strong desire to have male child.
  • During 1st month of pregnancy her mother was expired.
  • She was very attached to her mother She lost her weight at that time.

LIFE HISTORY OF PATIENT GIVEN BY MOTHER :-

MENTAL GENERAL:-

  • Timidity

PHYSICAL GENERAL:-

  • Hot ,
  • Thirsty ,
  • Desires pica ,
  • Aversion to milk +++
  • Desire for sugar +++ (not sweet).

FINAL PRESCRIPTION:-

Follow up after 1 month :- ( 16/08/ 2012)

  • Walking better ,
  • Do not fall down while walking ,
  • Hardness of calf muscle diminished ,
  • Loose motion for two days.
  • Difficulty in staring is still there,
  • Patient become more active,
  • Stool, Appetite ,Sleep normal

Follow up after 4 month :- (9/11/2012)

  • Not fall while Walking & running
  • Hardness of calf muscle diminished ,
  • Calf muscle become soft than before
  • Increased activity , ( even guest of his home notice that he was become more active than before & not fall )
  • Coryza started…….continous for 7 to 8 days. & then stop & again start coryza & stop intermittently.

Follow up after 7 month :- (09/02/2013)

  • CPK :-38,500
  • Gradually hardness of calf muscle diminished ,
  • Walk and run in clinic also not fall.
  • One incidence happened to him, fall down from chair, injury to forehead (cut) bleeding was there.
  • He had fever & pain (Ibujesic plus was given) & cut heal gradually. No pus formation was there.
  • Staring difficulty is still there but not like before
  • Slight improvement was seen.
